1.2选择药物不恰当对发作类型判断失误,未选用一线AEDs。

1.3药物剂量不恰当过小不能达到有效治疗血浓度,过大则诱使发作加重,口服药物后至少需要5～6个药物半衰期的时间方能达到稳态血浓度。因此,判断药物的最大最佳疗效应充分了解AEDs的推荐剂量和主要药代动力学参数。

1.4药物配伍不恰当药物代谢存在个体差异,肝酶诱导作用的AEDs(如CBZ、PHT、PRM、PB)联合应用加速药物代谢而不能达到有效血浓度。

1.5撤药发作关于停药的时间,一般的原则为:至少2年以上无临床发作、EEG无痫性活动;逐渐减药,撤至停历时3～5年以上。

1.6患者用药依从性差,或存在不良生活方式和习惯。

2真性难治

2.1原发性结构异常见于先天性发育不良和某些遗传性疾病。先天性发育不良:1局限性大脑皮质畸形。2灰质异位和多小脑回。3微小发育畸形。4一侧巨脑。

2.2继发性的病理因素包括各种后天获得性损伤或疾病。主要有:海马硬化、脑肿瘤、脑血管疾病、中枢神经系统感染性疾病或慢性炎症、免疫性疾病、代谢性疾病、慢性乙醇中毒、Alzheimer病、瘢痕或铁质沉积、钙化。
