1.停药的基本原则

首先患者至少2年没有出现过发作，成年人一般需要3-5年,其次要考虑患者停药后复发的危险性很小。这些条件是医生考虑患者能否停药的必备和先决条件。

2.停药后复发的危险因素

停药后复发是作出停药决定时必须要考虑的问题，与多种因素有关。主要的相关因素包括：(1)发病年龄：儿童发病者停药后预后大多良好，其他较差;(2)神经系统疾患：合并有神经系统疾患者停药后复发的危险性高于隐源性者;(3)脑电图情况：停药前脑电图异常比正常者复发的危险明显增高;(4)不同癫痫综合征的复发率明显不同：如青少年肌阵挛性癫痫药后复发率高，Lennox-Gastaut综合征的预后不好。另外，发作类型、严重程度、癫痫家族史、无发作期的长短等都可能对复发有影响。

3.何时停药与如何停药

对于经过多长时间的无发作期就可以考虑停药，比较普遍的看法是对于大多数患者平均3年的无发作期就可以了。但是，对于青少年肌阵挛性癫痫则至少需要5年。

4.停药前需要做哪些检查

一般停药前需要常规进行脑电图和抗癫痫药物的血药浓度监测。主要目的是帮助决策是否可以停药及制订停药方案。如果患者没有做过头部CT和核磁检查,在停药前必须行此项检查,以便了解颅内有无器质性病灶存在。
