第一,癫痫在少数情况下,有不定期遗传影响,对大多数影响不大,影响大小主要与病因有关.原发性患者亲戚中,患者发病率为3%—4%,继发性患者中为0—1%,说明原发性影响大,血缘关系越近发病率越高.父母双方均有癫痫或有一子女已发病,则第三代的发病率为20%,因此,原发病人虽可结婚,但应限制生育.癫痫病人选择配偶时,不要选择患过癫痫病的人或有家族病史的人,血缘关系越远越好.

第二,癫痫患者怀孕后,45%的人发作次数增加,特别是服用西药,通过肝脏代谢,胎儿组织和胎盘易致抗癫痫药血清浓度降低,而不能控制发作,若增加剂量,对母亲和胎儿都有影响.

第三,特别是抗癫痫西药,胎儿致畸发病率2.2%—13.8%.常见有腭裂,唇裂,心脏异常.致畸的发生率一方面与母亲的年龄,家族史,病史(如糖尿病)等有关,另一方面与所用药物有关,特别是有些西药.为了防止胎儿致畸,最好服用中药,症状控制应在3年以上,年龄不超过35岁.患者或家族中有畸形遗传病者,应不生育,患者有流产,死产或生产出异常婴儿的,再产应格外小心.
