癫痫活动灶的准确定位可使医生的注意力集中在切除癫痫病灶或阻断癫痫放电的扩散途径上。癫痫外科手术成功的关键是癫痫灶的准确定位。头颅CT与MRI扫描已成为癫痫病人检查的常规，特别是MRI扫描不仅要包括T1，T2像，还要进行Flair扫描和常规的海马冠状扫描，其主要目的是发现颅内异常结构病灶，排除肿瘤、血管病等病变，但仅20%的癫痫能够通过MRI或CT等结构影象检查确认病理灶定位，同时通常病理灶与癫痫灶不吻合或不完全吻合。无论是否存在结构性病灶，确定引起癫痫发作的功能性致痫灶是最重要的。
