现在认为真正难治性癫痫应符合下列条件：

(1)频繁的发作，应有有效的药物、足够的剂量，药物血浓度在有效范围内，不能控制发作，经更换药物或联合用药仍不能控制发作。

(2)失神及肌阵挛发作每日至少1次，其它类型发作每月至少4次。

(3)无进行性中枢神经系统疾病或占位性病变。

(4)治疗观察3个月至2年仍不能奏效者。
